Dear 5th grade families,
We hope this message finds you well. Fifth grade is a significant milestone and as you begin to anticipate closure of your child’s Elementary chapter, we have an important request, and we need your assistance.
When your child visits your pediatrician for their 11-year check-up, they will typically receive 2 immunizations: Tdap booster and the first Meningococcal. We would love to have a copy of these immunizations when they receive them. Texas DSHS requires that we have documentation of these immunizations by the time your child enters their 7th grade year. We typically begin requesting these in the middle of their 6th grade year through May with less than desired results. Any outstanding records by the beginning of 7th grade becomes urgent and stressful for everyone involved.
When you schedule your 11-year-old well visit be sure to ask about these vaccines and obtain a copy at the end of your visit. If your child has already been to their check-up and received these immunizations, please send us a copy. If you did not receive a copy, request a copy from your pediatrician or check your patient portal for an updated immunization record.
Please send these updated vaccine records to your current campus nurse, listed below.
We hope by being proactive vs reactive we can accomplish this requirement with more ease and set your child up for a smooth transition to the Junior School.

The Campus Nurse needs a copy of your child's immunization record whenever they receive any of the required immunizations at age 4 entering Pre-K, age 5 entering Kindergarten and age 12 entering 7th grade. Thank you!
